COURSE REQUEST INFORMATION
We ask that you complete course requests using TeacherEase if at all possible.
Options for pre-enrollment:
1. You can print out the paper enrollment form below and turn it in at the Middle School. There is a drop box outside the main front door for you to drop paperwork off at any time.
2. Forms can be picked up any time at the main entrance door. You may request that a paper enrollment form be mailed to your home address. You may request this by emailing firstname.lastname@example.org. Once you receive it and complete it, you can either mail it back to the school or you may turn it in to the dropbox at the main entrance.
3. You may pre-enroll through the student's TeacherEase login. Step-by-step instructions to help you with this process are attached below.
If you have further questions about enrollment for next year or need assistance, please email Mrs. Gage at email@example.com. We would be happy to assist you and answer any questions. Students or parents may also request a call back by calling (918)223-9596 and leaving a message. Calls will be returned in as timely a manner as possible.

Proof of Cushing Residence (lease/mortgage statement or current utility bill)
